Grand River Healthy Communities Collaborative Group
The Grand River Healthy Communities (GRHC) initiative is guided by a vision to change the way we work together to provide healthcare services, support, education and access to health and wellbeing in our community.
In March 2015, ninety-five community leaders representing over forty organizations in Brantford and area came together to provide a framework for priority setting and a basis for collective action. The Health Equity Forum was the first step in a process to identify issues and opportunities to change the context of health inequities for residents in our community.
The GRHC mandate is clear – we are committed to improving health outcomes for the people of our community based on enhancements that are actionable and rooted in collaboration with community partners.
Identifying and addressing priority issues from a community and inclusive perspective are central to the process of creating healthy communities in the Grand River area.
